The goal of the Shock, Organic Dysfunction and Resuscitation Research Group (SODIR) is comprehensive, innovative research into the areas of shock, acute respiratory failure, acute kidney failure, organic dysfunction, resuscitation and critical patient monitoring. In addition, the research group has a special interest in applying artificial intelligence as an instrument to find innovative solutions for critical patients.
The research line in critically ill patient monitoring, led by Dr.J.C.Ruiz, has as objectives the research in the application of echocardiography and other non-invasive or minimally invasive monitoring systems in the study of the pathophysiological mechanisms of the hemodynamic and cardiac dysfunction of the critical patient, and the study of microcirculation and its involvement in the pathophysiology of severe sepsis with multiorgan dysfunction.

SODIR has created the Clinical Research Unit (CRU) for to participate in the clinical trials promoted by the industry. This Unit centralizes all the clinical trials that are developed in the Department and allows a professional management of clinical trials, being a very positive and highly valued imput by the pharma companys. The Unit also give support to competitive research projects. The CRU has developed more than 50 clinical trials and research projects. The Unit is formed by nursing and doctors exclusively dedicated to clinical research (24 hours, 7 days/week).

The research line acute renal failure kidney focuses in extracorporeal replacement therapies and acute kidney injury.
Mainly attention has been focused on continuous renal replacement therapy (CRRT), to which absorptive therapies and hepatic replacement therapies have been linked. In relation to CRRT we have recently participated in the REGISFRA multicentric study, whose objective was to draw a map of CRRT in Catalonia. In addition, our attention is focused on projects of therapy optimization concerning type and dosage as well as in biomarkers that can early predict the requirement of CRRT in our patients.
In relation to acute kidney injury (AKI), the projects are based on the early recognition of patients at risk for AKI and on the prophylactic measures necessary to prevent these patients from finally developing an AKI.
IP: Marcos Pérez Carrasco
The line of research in acute respiratory failure has special interest in the study of the involvement of the interleukin 33 and its ST2 receptor in acute lung injury and its role as a possible therapeutic target. A model of acute lung injury in mice has been designed to show the prognostic efficacy of interleukin 33 and ST-2 plasma levels. Currently, SODIR is evaluating the possibility that treatment with anti-IL-33 antibodies may decrease the degree of inflammation in a murine experimental model of acute lung injury.
Another issue of special interest is the use of high flow nasal cannula supportive therapy. In this sense, SODIR has participated in a multicentric study, recently published in JAMA, analyzing the effect of high flow nasal cannula on postextubation respiratory failure.
Finally, SODIR is also involved in several multicentric international trials about the use of esophageal pressure to guide mechanical ventilation settings in patients with acute respiratory distress syndrome.

Extracorporeal membrane oxygenation (ECMO) replaces lung function and therefore allows patients to be active even if their lungs are not functioning.
The course, directed by Dr. Jordi Riera, brought together around 30 teachers specialised in ECMO from different disciplines.
