Mask exemptions in respiratory patients may have associated risks of severe COVID-19

An article in the European Respiratory Journal shows that patients with respiratory disease are at high risk for severe COVID-19.

An article prepared by the Respiratory Effectiveness Group (https://www.regresearchnetwork.org/" REG), in which Vall d'Hebron participates, concludes that patients with a respiratory disease are at high risk suffering from severe COVID-19. The Face masks, respiratory patients and COVID-19 study has been published in the European Respiratory Journal, one of the most prestigious international journals in the respiratory field.There are many false health news, hoaxes and biased information about the current COVID-19 pandemic that circulate on the network, and also in print and television. However, confusion and misinformation sometimes occurs through official channels. And it can have serious consequences.One of them is the exemption for respiratory patients from the use of a mask, mandatory in Spain and other countries both in shared outdoor and indoor spaces since May 2020. The Spanish Law of the BOE of Wednesday, May 20, 2020 establishes that "they are excepted of this obligation those people who present some type of respiratory difficulty that may be aggravated by the use of the mask and to those whose use is contraindicated for health or disability reasons".At the REG, an independent global group of doctors and researchers in Spain, the United Kingdom, the United States, Canada, and Australia, among others, have reviewed the available evidence.The published article concludes that there is no evidence to support this exception, and that patients with a respiratory disease are at high risk of severe COVID-19, in Spain, the United Kingdom and other countries where these exemptions apply. This exception must be eliminated.Dr. Joan B Soriano, epidemiologist at the Pneumology Service of Hospital La Princesa in Madrid, member of the COVID-19 Clinical Network Knowledge Exchange Team of the WHO in Geneva, and lead author of the study declares that "patients with asthma, COPD or other respiratory diseases should wear the mask without exceptions. If they have respiratory failure or a feeling of shortness of breath when putting it on, it is best to limit their activity, especially outdoors".Not using a mask to avoid an asthma attack, or exacerbations of COPD, or other respiratory diseases, due to an alleged increase in inspiratory pressures through a mask, is unfounded.Dr. Sinthia Bosnic-Anticevich, from the University of Sydney Woolcock Institute, president of REG and also co-author, states that "if you or a family member of yours suffer from asthma, COPD or another chronic respiratory disease, wear a mask to protect yourself and protect others. This is a new virus and a new disease, but the information is clear and individual decisions are essential to defeat this pandemic".Dr. Marc Miravitlles, pulmonologist at the Vall d'Hebron University Hospital and researcher of the VHIR Pneumology research group, indicates that "in the absence of an effective vaccine, we must maximize prevention measures, including the appropriate use of masks. This also and especially includes respiratory patients".There are an estimated 545 million people worldwide with chronic respiratory disease, and not wearing a mask can carry an increased risk of personal and group infection.
